Certificate Reissuance

Authorized Circumstances for Reissuance:

1. Legal Name Change
   • Requires official documentation (e.g., marriage certificate, court order, government-issued ID)
2. Clerical or Administrative Error
   • Includes errors in name, course title, or credit calculation attributable to LER (including misspelling or omission of a last name OR UTC time stamp which does not reflect the student’s location)
3. Loss or Damage
   • Replacement of an existing certificate
4. Student not fulfilling all of the requirements for a certificate release
   • Not completing a lesson within a bundle of courses
   • Not completing the final exam
   • Not completing the final evaluation 
5. System or Accreditor-Driven Updates
   • Reformatting due to system transitions or accreditor requirements, without altering original credit or completion data
